Our practice welcomes patients from most areas of ML2 (not ML2 9) and ML1 (5). Our area includes Wishaw, Cleland, Overtown, Waterloo, Garrion Bridge, and Bogside.
Wishaw practices in the Houldsworth Centre operate a registration rota for new patients. Please contact NHS Lanarkshire reception on 01698 355511 to find out which GP practice is accepting new patients that week.
To register permanently with a GP Practice please complete the below New Patient Registration form, Health Questionnaire and Online Services form. Alternatively, forms can be collected at GP Reception on Level 3. All registration forms must be returned by 5pm Friday of the week we are open. Please answer all questions within each form.
You will require to provide identification when registering with the practice, if possible, please include copies of your identification with your registration form. Completed forms and ID can be emailed to Logan.Reception@lanarkshire.scot.nhs.uk before 5pm Friday of the week we are open.
Please note any registration forms emailed on the weeks we are closed will not be processed.
Consent form for Online Services
Once you have been registered with the practice you will receive an email asking you to create a password for an askmyGP account. For more information on askmyGP please click here.
If you have requested an online account for ordering your medication and online services you will receive this around a week after registration. For more information on Patient Services ordering, please click here
Please ensure you have sufficient medication from your previous practice as we do not have access to your medical records/repeat prescription information straight away.
Please register your newborn baby as soon as possible, you don’t need to wait until their formal registration at the civic centre.
Temporary patients
If you are not registered with the surgery you will be unable to see one of our doctors unless you have a serious emergency. In most other cases, calling 111 is a better option.
If you are not registered but need treatment at the surgery you will need to complete a temporary registration form.
You can be registered as a temporary patient for up to three months. This means you can be registered with our practice and remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with our practice.
