We currently offer a text message service to our patient’s. Information provided in the message will vary from appointment details, advising patient to contact the surgery or askmyGP, asking patient to arrange an appointment with the nurse, etc.
To opt into this service download and complete our consent form and return it to the practice using our red box situated on the ground floor of the building.
Not available for under 16 years of age.
You may have already provided consent previously, but we would appreciate it, if all interested parties, would complete the form to allow us to up date details.
At the moment, email message service is not available. You can, however, provide consent which will enable us to contact you in this manner, should the service become available.
It is the responsibility of the patient to keep their contact details updated with the practice
