Updating your details

If you change your name, address or contact information, please update your details as soon as possible. A form can be collected from GP Reception or you can download, complete and email back our Change of Personal Details form.

Address Changes

Please note we only accept patients from Wishaw (not including Newmains), Cleland, Waterloo, Garrion Bridge and Bogside. If you are out with our practice boundaries, you will be required to register with a GP in your local area within 4 weeks, after which time, you will be removed from our practice list. Any requests to change the address of a child under 16 years old without any parent changes, will be investigated further by Logan Practice. We will not change any details until there is confirmation that the child alone has to be changed.

Name / Gender Changes

Proof of name change will be required for anyone under the age of 16. This can either be deed poll documentation or new birth certificate. For any transgender patients wishing to also change their sex, please tick the relevant box on out change of details form. Practitioner Services will then contact that patient to confirm change, and new CHI number will be issued. Unfortunately, CHI numbers cannot currently be changed to record non-binary gender identity. Non-binary patient’s will be registered under their birth gender, unless they request a change.

Change of contact information

It is the responsibility of the patient to ensure their contact information is up to date. Logan Practice will not be held responsible should any e-mail or text message be viewed by a third party. No identifiable information will be entered into either method. If using a family member’s information, patient should be aware, that named family will see information sent by Logan Practice. If you wish to consent to text and e-mail correspondence, please ask a member of staff for consent form.
